Position: Jordan’s Principle Case Worker Off-Reserve
Bloodvein River Health/Bloodvein First Nations
​
Location: Winnipeg, Manitoba.
​

Under the supervision of the Jordan’s Principle Director and in accordance with Bloodvein First Nation Mission, Values, Goals, Policies, and Procedures, the assistant case worker will be responsible to assist all off-reserve requests for the Bloodvein First Nation peoples: will provide assistance, follow up, and will liaise with other Jordan’s Principle Off reserve offices. Preference for Bloodvein Band Members before others.

Key Responsibilities:

Assist Off-reserve Case Worker with daily tasks.

Accompany other case workers on home visits when necessary.

Case Worker will do all relevant documentation.

Develops and documents a service plan that includes referral, assessment, treatment/admission, discharge (if applicable), transition, and ongoing protocols for the families.

Ensures the appropriate and effective sharing of information including identifying and maintaining contact with First Nation children and families as required for continuity of care, including the use of multiple communication strategies including tele-health.

Assist in finding resources for children and families.

Understand the importance of working with First Nations protocols to make children and families comfortable with their interaction with the health care system & other systems.

Build strong relationships and partnerships with front-line health care and childcare workers, and with a variety of health care and childcare professionals, including nurses, speech pathologists, dental, occupational therapists.

Provides community-based services like workshops, presentations, and team-building activities focused on developing and enhancing the skills and capacity of front-line workers.

Willing to work after hours and/or weekends as required/deemed necessary.

Able to assist with land-based cultural activities with youth.
Qualifications: (including, but not limited to)
-
Bachelor of Social Work, RN, LPN, RPN, B. ED Post-Secondary preferred or a combination of related work experience and/or training.

Knowledge of Jordan’s Principle history and why this legislation exists.

In-depth knowledge of First Nations health and wellness system needs.

Knowledge of relevant legislation such as the CFS Act, Vulnerable Persons Legislation

Highly Organized with Great Attention to detail
-
Must have excellent written and communication skills.

Proficient in Microsoft Office
-
Ability to work independently.

Good Organizational, time management and prioritizing skills
Conditions of Employment:

Valid Manitoba Driver’s License

Current Criminal Records/ Vulnerable and Child Abuse Registry Checks

Access to a reliable vehicle

Must be willing to travel as required/requested.
Salary: Dependent Upon education and experience
